Session spotlight: Simplifying the upper quadrant: neck, shoulder, and upper back pain with Michael O'Reilly
Michael is a musculoskeletal (MSK) Physiotherapist with over 20 years of experience specialising in the assessment and management of neck pain, headaches, shoulder pain, and complex upper quadrant dysfunctions. Throughout his career, he has remained passionate about empowering patients with the knowledge, tools, and confidence to move optimally in an increasingly sedentary and posturally demanding world.
During an engaging and thought-provoking session at Therapy Expo 2024, Michael introduced delegates to his innovative approach to treating upper quadrant dysfunction, centred around his creation: the Balance Wedge. Combining decades of clinical expertise with biomechanical principles, he demonstrated how simple interventions can help address the growing impact of modern technology on posture and movement.
Understanding the root cause
Michael encouraged delegates to think about upper quadrant dysfunction using the analogy of a tree. Rather than focusing solely on symptoms, he highlighted the importance of identifying and addressing the underlying causes.
The roots represent our "stone age" bodies attempting to function in today's "information age", where prolonged screen use and digital overload place unprecedented demands on our posture.
The trunk symbolises the sustained static positions we adopt throughout the day. Over time, these prolonged postures fatigue the body's core stabilising systems and create biomechanical inefficiencies that feed dysfunction.
Finally, the branches are the symptoms clinicians see every day, including neck pain, headaches, shoulder discomfort, upper thoracic stiffness, and restricted movement.
Digital overload and the modern posture crisis
Reflecting on the average amount of time people now spend using smartphones, tablets, and computers, Michael explained how repeated forward-flexed postures create significant mechanical stress throughout the upper body. This can lead to compensatory overload of the neck and shoulder girdle, increased strain on myofascial and joint structures, and a gradual cycle of pain, stiffness, and reduced mobility.
To illustrate this concept, he used the analogy of a filing cabinet that is constantly being pulled forwards. As the cabinet tips, greater effort is required to keep it upright. Similarly, poor posture continually increases the demands placed on muscles and joints, eventually leading to dysfunction if left unaddressed.
Biomechanical ratchets and wedges
A key focus of the session was understanding the biomechanical structures that contribute to postural imbalance.
Michael identified the Pec Minor as one of the body's primary "ratchets". When shortened, it draws the trunk forward, alters shoulder mechanics, and can compress important neurovascular structures.
The Upper Trapezius, meanwhile, often becomes overactive as it attempts to compensate for poor trunk positioning. Michael emphasised that lasting improvements require coordinated treatment of both muscles rather than focusing on one area alone.
The Balance Wedge was presented as a practical tool to help restore more balanced posture, improve trunk positioning, and support more effective movement patterns alongside targeted rehabilitation exercises.
